Re-enlistment Application

This form is for musicians who have previously performed with our group and wish to be considered for future projects and performances.

Filling out this application does not guarantee involvement in the group for any or all projects/performances. Please read the following information, then fill out the form if you would like to apply for the re-enlistment period. We will select who will play for performances going forward based on skill level, reliability, loyalty, availability, and other factors.

January 7th, 2026 through June 20th, 2026

5/17/25 - Spirits of the Wind: A Night of Ghibli Music (At The Depot SLC)

6/23/25 - Spirits of the Wind: A Night of Ghibli Music (At the Viridian Event Center)

Violin I - 8 to 12

Violin II - 8 to 12

Viola - 6 to 10

Cello - 6 to 10

Double Bass - 2 to 4

Flute/Picc - 2 to 3

Clarinet - 2 to 3

Oboe - 2

Bassoon - 2

Contrabassoon - 1

Horn - 2 to 4

Trumpet - 3 to 4

Trombone - 3

Tuba - 1 to 2

Perc - 3 to 6

Additional instruments may be called upon for specific projects or concerts.

Compensation:

Varies from show to show. Expected anywhere from $0 to $200 per show.

Media Release

By participating in activities of the Deseret Symphony Orchestra, I consent to its representatives, agents, subsidiaries, related entities and assigns permission to take and create photographs, images, videos, recordings, quotes, summaries, biographies, and/or excerpts of and about me. I give permission to the Deseret Symphony Orchestra to use and re-use, publish, distribute, and/or broadcast the Media, in whole or in part, individually or in conjunction with other photographs, images, videos, text, quotes, summaries, biographies, and/or excerpts, in any and all promotional, educational, business, trade, advertising, and marketing materials, including, but not limited to, in connection with product endorsements. These materials may include e-mails, the Internet, social media, websites, and other electronic media, as well as traditional radio, television, cable television, newspaper, magazine, direct mail, catalogs, brochures, advertisements, promotional materials, press releases, training materials, and other uses, in such medium and formats (whether now known or hereafter created or invented), at such locations, at such times and for such purposes as Deseret Symphony Orchestra chooses, in its sole discretion. I hereby waive any right that I may have to inspect or approve the finished product or products or the advertising copy or printed matter that may be used in connection with the Media or the use to which the Media may be applied. In addition, I waive all rights that I may have to any claims for payment of any kind, whether money, property, or otherwise, related to the recording, production, publication, use or re-use of the Media. *

Sheet Music Anti-Distribution Agreement

SHEET MUSIC ANTI-DISTRIBUTION AGREEMENT: By signing this agreement, I acknowledge and agree to the following terms regarding the use of sheet music provided by the Deseret Symphony Orchestra (DSO): I will not print, download, or use the sheet music for any purpose other than performing with the DSO. I will only print or download the parts assigned to me unless I have explicit permission to access additional parts. I will not share, distribute, or otherwise redistribute DSO sheet music to anyone outside the orchestra or for any purpose unrelated to DSO performances. I understand that these guidelines are in place to protect the intellectual property and integrity of the orchestra's materials. By typing my name below, I agree to abide by these terms.

Committee Position Agreement

COMMITTEE POSITION AGREEMENT:

As a member of the Deseret Symphony Orchestra (DSO), I understand that our success depends on the contributions of each individual. To support the orchestra’s operations and growth, I agree to select a committee position and do my best to fulfill the responsibilities associated within. I recognize that active participation in a committee is an important part of contributing to the orchestra community, and I will strive to collaborate effectively with my fellow members to achieve our shared goals.
